Agenda
This Conference is designed to convene providers, community-based organizations, payers and other leaders that are actively developing strategies and tactics to improve the care transition process. Learn from national coalitions as well as get regional and local perspectives on innovative strategies to improve transitions in care, with a focus on participation, action, and collaboration. Innovators from across the care continuum will share their insights and expertise on person-centered, effective practices and methods for overcoming transitional care barriers. A combination of plenary speakers, panel sessions, and networking time will provide multiple opportunities for interacting and translating ideas into action.
